When should I consider Hospice? Consideration for hospice care has always been a very difficult decision for Physicians to make. Families all to frequently will not create the impetus or understand the complexities that go along with making such a recommendation. General guidelines exist for various illnesses to help guide you the Physician along this path. Criteria that are used will be presented in latter discussions for disease specific models.

A mistaken notion has existed in the medical community that hospice care should be rendered when the patient is in his/her final hours of life. This approach misses the months leading up to that moment when patients and families most need that multifaceted support in their home setting. Our Medical Directors and Team Leader Nurses seek to educate and assist the Community Physicians in determining a patient’s appropriateness for hospice care.

If you have any concern for any patients or think they may benefit from our services, please utilize the contact information provided below so that we may assist you in caring for the patient.
